Scheduling Details and Forms

To schedule your initial in-person "Wellness Dicussion" with Nancy Lonsdorf MD please go through the following steps.

1. Fill out and submit the health history questionnaire:
Upon submission you will be redirected to Dr. Lonsdorf's online scheduling service where you can pick and schedule your appointment.

2. Schedule an appointment:
This online scheduling service will allow you to choose an appointment, cancel or change an appointment and send you confirmations and reminders of your appointment. You can also go directly to this scheduling page by clicking here.

3. Download the following documents:

A. Dr. Lonsdorf's Practice Brochure - Download PDF. (You may also read this online by clicking here) This provides the basic information on Dr. Lonsdorf's services.

B. Prevention Handout - Download PDF: Describes the various prevention tests and checkup options available and recommendations for when to use them.

C. Wellness Authorization Form - Download PDF. Please read and sign this form if you agree with the conditions of the Wellness visit. Then scan and email to drlonsdorf@gmail.com or fax to 315-546-2146.D.

D. Ama Quiz - Download PDF: Please fill

D. Optional Directions - Download PDF:If you are not familiar with where Dr. Lonsdorf has her office in the Rukmapura Hotel in Maharishi Vedic City then please download the directions here.
