Scheduling at a Glance Follow these 3 Simple Steps: 1. First, please download the five documents you listed below. You will need to fill out and return these health information forms before your appointment. 2. This visit is for people who have had a visit with Dr. Lonsdorf before either in person or by phone. To schedule your follow up visit with Dr. Lonsdorf please complete the following steps.
1. Download the following 5 documents:
A. Dr. Lonsdorf's Practice Brochure - Download PDF. (You may also read this online by clicking here) This provides the basic information on Dr. Lonsdorf's services.
B. Established Client Information Form:
FIll out one of the following two informational forms
Repeat Evaluation Info Sheet: If you have had an in-person visit with Dr. Lonsdorf since August 2005 (i.e. you've consulted with her at her office at the Rukmapura Park,) please download and fill out the Repeat Evaluation Information Sheet. After filling out, please fax this to 315-546-2146 or scan and email to drlonsdorf@gmail.com. You may also bring it with you to your appointment. Download PDFOR
Online Questionnaire: If you have not had an in-person visit with Dr. Lonsdorf since August 2005 (at her office at the Rukmapura Park,) then please fill out the online Health History Questionnaire. When you finish filling out the questionnaire you will automatically be sent to the online scheduling service to make your appointment.

2. Schedule an appointment:
Dr. Lonsdorf's online scheduling service will allow you to choose an appointment, cancel or change an appointment and send you confirmations and reminders of your appointment. You can go directly to this scheduling page by clicking here and you will also be automatically sent there when you fill out the online health history questionnaire (for those who are doing a phone visit or have not seen Dr. Lonsdorf since August 2008.)
3. Payment:
Payment will be taken at the time of service. Checks or credit cards are acceptable. (See consultation fees listed above.)
