Scheduling Details and Forms

To schedule your initial phone visit with Nancy Lonsdorf MD please go through the following steps.

Initial medical phone visits are only available to women living in Iowa, North Carolina or New York.

Non-Medical Wellness Education Discussions are available to women in other states - click here for details.

1. Fill out and submit the health history questionnaire:
Upon submission you will be redirected to the page you can pick and schedule your appointment.

2. Schedule an appointment:
This online scheduling service will allow you to choose an appointment, cancel or change an appointment and send you confirmations and reminders of your appointment. You can go directly to this scheduling page by clicking here.

3. Download the following documents:

A. Dr. Lonsdorf's Practice Brochure - Download PDF. (You may also read this online by clicking here) This provides the basic information on Dr. Lonsdorf's services.

B. Prevention Handout - Download PDF: Describes the various prevention tests and checkup options available and recommendations for when to use them.

C. Checklist of what to send Dr Lonsdorf before your appointment - Download PDF.

D. Optional Directions - Download PDF:If you are seeing Dr. Lonsdorf in-person and are not familiar with where Dr. Lonsdorf has her office in the Rukmapura Hotel in Maharishi Vedic City then please download the directions here.
